上肢静脉抽取的血标本;有时需要动脉抽取血标本,特殊情况下脊柱穿刺取脑脊液。

检测样本
检测什么?
该项目检测的是血液中(很少情况下是脑脊液中)乳酸盐的含量。乳酸盐是乳酸的离子(携带电荷)形式。它是肌肉细胞、红细胞、脑及其它组织无氧酵解的产物,通常血中含量处于低水平。机体首先进行的是有氧呼吸,但是它需要提供充足的氧气。有氧呼吸发生在线粒体中,线粒体是机体每个细胞内的小小产能站,能够利用糖和氧气来产生机体的初级能量源ATP(三磷酸腺苷)。
然而,当细胞内含氧量下降和/或线粒体功能异常时,机体必需转向能效较低的无氧酵解过程来进行糖代谢以产生ATP。乳酸是这一过程的主要副产品,其累积量会很快超过肝脏对其降解量。当血液中乳酸水平显著增高时,患者最初会出现高乳酸血症,而后会出现乳酸酸中毒 (LA)。通常机体能够对高乳酸血症的影响进行代偿性调节,但是乳酸酸中毒则往往会非常严重以至于破坏人体的酸碱平衡,导致诸多临床症状如:肌无力、呼吸急促、恶心、呕吐、出汗甚至昏迷。
乳酸酸中毒分为两型:A型和B型。A型是由于肺的氧摄取不足和/或低血流量(血流灌注不足)导致输送到组织的氧气减少。最常见的原因是包括外伤和失血在内的不同原因导致的休克,但是乳酸酸中毒也可见于心脏病发作、充血性心力衰竭和肺水肿(肺内液体积聚)等疾病状态。B型是由于某些疾病状态导致血液中的乳酸盐/乳酸累积,与有效氧降低无关。这些疾病状态包括肝脏和肾脏疾病、糖尿病、白血病、艾滋病、糖原贮积症(如葡萄糖-6-磷酸酶缺乏症)、药物和毒素、严重感染(全身性脓毒症和脑膜炎),及多种遗传性代谢和线粒体疾病(影响ATP正常生成的肌营养不良的一种)。高强度运动也会导致血液中乳酸水平升高。
测试样本如何采集?
血液样本由上肢静脉穿刺获得。有时需要动脉穿刺获得动脉血样本。特殊情况下需要通过腰椎穿刺从脊柱获得脑脊液样本。
是否需要进行任何试验前准备以保证样本的质量?
可能需要或不需要空腹。咨询自己的医生。也有可能需要检验前一段时间禁止运动。
