检测什么?
此试验检测血液、尿液或肝脏中铜的含量。铜是体内合成酶类必不可少的矿物质,这些酶在调节铁代谢,构成结缔组织,在细胞水平产生能量,生成黑色素,神经系统和脑组织功能中发挥重要作用。
许多食物含有铜,包括坚果、巧克力、蘑菇、贝类、全麦、干果和肝脏。使用铜管运输饮用水使饮用水中含有铜,使用铜盘烹饪和盛放食物使食物中含有铜。通常,铜在肠部吸收,与蛋白结合使其无毒,运输到肝脏。肝脏储存部分铜,其余大部分与铜蓝蛋白前体结合,一旦结合铜,成为铜蓝蛋白。血液中大约95%的铜与铜蓝蛋白结合,肝脏将过量铜分泌到胆汁,从粪便排出体外。一些铜也可从尿中排出。
铜过量和铜缺乏都非常罕见。Wilson病,一种遗传性代谢紊乱,造成铜在肝脏、脑组织和其它器官过量蓄积。这会导致组织损伤,出现以下体征和症状:
如果肾脏受损,出现少尿和无尿。这些症状还见于急性或慢性环境暴露导致的铜中毒,或患有肝脏疾病或障碍阻止或抑制铜代谢和排泄。
严重吸收不良,如囊性纤维化和腹部疾病,牛奶配方喂养婴幼儿,有时候会出现铜缺乏。症状包括嗜中性粒细胞减少,骨质疏松和小细胞性贫血。一种非常少见的X-连锁遗传性疾病称为Menkes卷发综合征,导致患病婴幼儿脑组织和肝脏铜缺乏。此疾病主要影响男性,与癫痫发作、发育延缓、脑组织动脉异常发育和灰色易扭结发(罕见)有关。
